This full-time, 5-day-a-week position offers the perfect blend of independent practice and robust organizational backing. Because our compensation model rewards your dedication, you can expect an estimated annual income of $140,000 to $170,000, with absolutely no ceiling on your productivity bonuses. You will set your own schedule and round on 15-20 patients a day. Paid malpractice insurance as well as a full comprehensive benefits program including time off.

Your Impact
As a vital member of our specialized regional team, you will champion the health of seniors during critical transitional phases of their lives. You will transition away from rushed, transactional appointments. Instead, you will build deep, lasting relationships with residents and their loved ones while designing paths to recovery alongside a dedicated medical staff.

Daily Scope of Practice

·        Run independent clinical rounds, evaluating complex patient conditions and tracking recovery milestones.

·        Design and pivot customized therapeutic plans tailored to unique patient goals.

·        Direct transitional care strategies to minimize hospital readmissions.

·        Act as the clinical anchor for families, translating complex medical data into clear, reassuring guidance.

·        Keep precise digital health records that meet modern compliance standards.

What You Bring to the Team

·        Valid Connecticut NP or PA credentials alongside active DEA registration.

·        A passion for geriatric or transitional medicine; we highly value experience in ER, acute care, or primary clinics, but we happily provide training for motivated new graduates.
