    Short Description

    • Linkage to care point person for newly identified HIV infected persons enrolling in Cooper EIP medical care services.
    • Re-engage HIV infected persons lost to care. Provide partner services to new and existing HIV infected patients.
    • Work collaboratively with the medical care team providing patient counseling for management of medical treatment care plan.
    • Employ social networking strategies with new and existing HIV-infected patients to bring additional social contacts in for HIV testing.
    • Provide prevention counseling/strategies such as CLEAR, ARTAS, or Partners in Health for patients and their families.
    • This position's role is key in the implementation of multi-disciplinary care.

    Experience Required

    Minimum of 1 year experience in providing HIV prevention or care services to people living with HIV. Experience counseling HIV (+) clients on connecting to available services. Knowledgeable of community resources.

    Education Requirements

    BSN required, current NJ RN License.

    License/Certification Requirements

    Current NJ-RN License, BSN required.
