Responsibilities:

Care Management is a team-based, patient-centered approach designed to address the increasing complexity of care in outpatient settings and for those recently discharged from an acute care setting. Through collaboration across ambulatory, inpatient, and community-based settings, care management is intended to improve patient outcomes and reduce the need for medical services by helping patients and caregivers more effectively manage health conditions, address health related social needs, and better navigate the heath care continuum. Core programs in the Care Management department include Chronic Care Management, Advanced Primary Care Management, Transitional Care Management, and Care Navigation.
The nurse manager is responsible for the oversight of Care Management activities for the Bassett Healthcare Network. The manager plans for and secures financial and human resources for the department. Maintains oversight responsibility for the programs designed to be implemented by the care management department by both clinical and non-clinical resources. Responsible to assist with education on the implementation of clinical protocols. Leads care management programs designed to support population health objectives and managed populations. Manages quality improvement activities and oversees the competencies within the department

 

Essential Job Functions

  • Develops and monitors operational procedures and performance standards which ensure safe, effective and efficient daily operations.
  • Develops and monitors departmental policies and protocols which meet the needs for the provision of Care Management services.
  • Develops strategies to ensure effective and efficient departmental performance and service delivery.
  • Drives continual optimization and utilization of EMR best practice and work flow development. 
  • Monitors and reports departmental performance related to established objectives and goals.
  • Develops staffing and scheduling to meet demands for operational services.
  • Provides direction to ensure appropriate use of staffing resources.
  • Develops and implements plans for staff development, orientation and professional growth.
  • Completes annual performance appraisals for all staff within established timeframes
  • Manages and participates in staff recruitment, interview process and selection.
  • Provides timely and effective communications with department staff to maintain awareness of important corporate, division and departmental information.
  • Schedules and directs staff meetings on a regular basis.
  • Makes use of staff feedback and insight to improve services.
  • Participates in multidisciplinary committee work groups and teams to support and facilitate initiatives and programs for service improvement.
  • Develops and implements annual operational budget and capital budgets.
  • Monitors and reports budgetary variances and emergency capital needs.
  • Reallocates, prioritizes and, when indicated, recommends modifications to established budgetary plans.
  • Develops and implements expense control measures to ensure financial objectives.
  • Works to develop and maintain revenue generating opportunities
  • Takes action on identified areas for performance improvement by implementing plans and evaluating effectiveness of improvement measures.
  • Tracks and reports performance improvement activities as appropriate and/or required by institutional guidelines.
  • Ensures compliance with procedures, policies and regulatory requirements. Implements departmental policy.
  • Ensures compliance with all regulatory agency requirements.
  • Ensures compliance with departmental and institutional policies.
  • Proactively identifies safety-related issues, addresses identified issues and institutes procedures which minimize risks to patients and staff.
  • Reports and acts to correct compliance and risk related issues
  • Development of Bassett Network/System Thinking - A leader who thinks globally and adopts philosophy of continuous improvement for Bassett processes/systems and the Bassett Network.
  • Develops working relationships with partners throughout the Bassett Network to effect positive results.
  • Regularly assesses opportunities to enhance system(s) performance
  • Develops action plans and completes them to improve patient and employee satisfaction
  • Connects with counter parts at affiliate sites to actualize potential for adoption of best practices and standardization
  •  As appropriate, takes action to create a better experience for patients traversing the continuum of care.
  • Measures deployment of resources and develops plans to optimize ROI.
  • Performs other duties as requested and observed by Director of Population Health & Care Coordination

 

Qualifications:

Education:

  • 4 Year / Bachelor’s degree in Nursing or related healthcare field required

Experience:

  • Minimum 3 years of management experience, required
  • Minimum 2 years of work experience in care/case management, in the acute care or ambulatory nursing environment or care coordination required

Licensure/Certifications:

  • Current NYS Registered Nurse License, required

Physical Requirements:

Total Rewards:

We are also offering a sign-on bonus for this position! The bonus amounts are based on the position type and are listed below. Additional details and eligibility requirements will be discussed during the interview process. Current employees of Bassett Healthcare Network are not eligible and are encouraged to apply through the internal job board. At the discretion of leadership, this program may terminate or be suspended at any time. 

 

Bonus Amount:

  • RN (excluding management) sign-on bonuses are awarded in the amount of $20,000 for full time
  • LPN sign on bonuses are awarded in the amount of $10,000 for full time
