University of Iowa Health Care Department of Nursing is seeking a 50% Adolescent and Young Adult Nurse Navigator to work for the Holden Comprehensive Cancer Center with the AYA Team.

Summary of Job Responsibilities
Functions as a clinical expert and care coordinator for a specific patient population interfacing with administrative, clinical nursing staff, physicians, and patients/families. The Adolescent and Young Adult (AYA) Nurse Navigator functions as a clinical expert and care coordinator for AYA Program of the Holden Comprehensive Cancer Center (HCCC). The AYA Nurse Navigator functions as a clinical liaison between the primary healthcare team and patients/families with attention to the unique experiences of AYA cancer patients, including psychosocial care, onco-fertility, and clinical trial enrollments. They will provide seamless patient-centered navigation through the continuum of care from initial diagnosis until transition into survivorship or end-of-life care.
Position Responsibilities:
• Serves as a clinical expert and resource to AYA patients with a cancer diagnosis ages 18 to 39. • Meets with patient at enrollment to AYA program to make a face-to-face introduction and provide navigation-related education. • Places follow-up orders for routine check-ins with AYA patients throughout their cancer trajectory to educate and access for unique needs. • Communicates with physicians and members of the healthcare team directing patient care. • Collaborates efforts with multidisciplinary team members to streamline fertility preservation process and sexual health. • Works in partnership with the patient's primary cancer treatment team to ensure patients and family members receive streamlined information. • Partners with the multidisciplinary team, including social work, psychosocial support, financial counseling, as well as others, to navigate a smooth patient experience. • Collaborates with nursing staff and other healthcare professionals in the development and review of standards and in planning, implementing, and evaluating patient care for AYA patients. • Coordinates referrals with internal and external physicians. • Provide leadership for AYA patient care programs and collaborate with unit-based nurse management teams. • Participates in development and implementation of managed care programs for AYA patients. • Advocate for the unique needs of AYA patients, families, and staff. • Serves as an educational and clinical resource to AYA patients/family/staff (supports patient/family/educational activities). • Facilitates learning and provides education to those in external agencies, communities, and to other healthcare professionals. • Educate respective departments on the unique needs of the AYA program. • Promote program awareness and connectiveness between various UI Health Care departments and divisions. • Interacts with staff, patients, medical staff, peers, and others in a professional and cooperative manner. • Participates in the development of goals for the AYA Program. • Provides clinical triage for calls made to the AYA Program. • Provides content for monthly AYA newsletter. • Represents AYA in monthly Teen Cancer America webinars and other national committees. • Attend meetings and participates on committees and support groups as appropriate; serves as a member of the AYA Core team and AYA Steering Committee. • Assists patients in completing AYA Enrollment form and documents relevant information. • Maintains EPIC documentation and metrics of all AYA enrolled patients. • Shares pertinent information with AYA Patient and Family Life Specialist, AYA Social Worker, and other team members on a continual basis regarding AYA enrolled patients. • Performs all other duties and tasks assigned as they relate to patient/family education, care coordination, nursing SOPs, nursing quality and improvement, and nursing education. • Adheres to professional and legal standards of nursing practice. • Maintains confidentiality of patient and staff information. • Promotes a positive work environment. • Exhibits a commitment to quality and process improvement. • Remains calm and constructive in conflict situations, listens, and attempts to understand other points of view. • Performs other special projects and tasks as assigned.
Requirements and Benefits
Percent of time: 50% Schedule: Monday-Friday flexible daytime hours. No weekends, No Holidays. Salary: $32,121 minimum at 50% effort to commensurate. Salary will be based on years of experience and percent effort. Required Qualifications
A Baccalaureate degree in Nursing is required. Current license to practice nursing in Iowa is required by date of hire. Reasonable (within the last 1-3 years) nursing experience is required. Demonstrates expertise in specific clinical area of practice.
