Job Summary:

Looking for a nursing role where you can build relationships, improve lives, and avoid the demands of bedside nursing?

Join our Adult Community Services team as a Nurse Care Manager and help individuals achieve better physical and behavioral health outcomes through care coordination, wellness coaching, and community-based support.

What You'll Do:

  • Coordinate healthcare services for adults receiving community-based support
  • Conduct health screenings and nursing assessments
  • Develop individualized wellness plans
  • Connect individuals with medical and behavioral health resources
  • Educate clients on wellness, prevention, and self-management
  • Collaborate with providers and treatment teams
  • Track health outcomes and maintain documentation

What We're Looking For:

  • Active Missouri RN or LPN license
  • Passion for helping people improve their health and independence
  • Strong communication and relationship-building skills
  • Experience in care coordination, community health, behavioral health, case management, or related nursing services preferred

Additional Qualifications:

  • Current driver's license, acceptable driving record, and current auto insurance required
  • Successful completion of background screening, including criminal record, driving record, abuse/neglect, and fingerprint checks
  • Ability to establish productive working relationships with participants, healthcare providers, community partners, and team members
  • Demonstrated leadership skills with the ability to provide guidance and support to Wellness Coaches
  • Commitment to person-centered care, teamwork, and continuous quality improvement
