    Job Description

    Summary

    The Nurse Care Manager (NCM) is responsible for lateral integration of care across all settings in which healthcare is delivered to his/her assigned patients. Through the assimilation and application of evidence-based information, the NCM designs, implements, and evaluates client treatment plans by coordinating, delegating and participating in the care provided by the health care team. The NCM is responsible for working within the health care system to promote an environment that will empower his/her assigned patients to manage their illness(es) and achieve optimal outcomes.

    Minimum Requirements

    • BSN or the equivalent in a combination of an ADN and relevant experience

    • 1-2 yrs nursing experience in a behavioral health setting

    • Current Missouri License as a Registered Nurse

    • Minimum of three (3) years experience working in a healthcare setting

    • Excellent written and verbal communication skills

    • Must be able to read and speak the English language

    • Proficient in Word, Excel and Powerpoint

    • Current BLS

    • Driving is a requirement for this employee; he/she must meet the requirements specified in the Fleet Driving Safety and Compliance Policy and must continue to meet these requirements as a condition of employment

    Preferred Requirements:

    • Community nursing service experience

    • Multi-language skills
