Provide skilled in-home nursing care for low- to high-acuity patients
Perform comprehensive patient assessments and follow individualized plans of care
Administer medications and treatments as prescribed
Collaborate with families and interdisciplinary team members to support positive patient outcomes
Accurately document care and communicate patient changes to the RN Clinical Supervisor/Site Director
Safely lift, position, and transfer patients, supplies, and equipment
Active Arizona RN or LPN license, or Compact License
Valid Arizona Class D Driver’s License
Current BLS Certification (online-only courses not accepted)
Current TB test or chest X-ray within the past 12 months
Level One Fingerprint Clearance Card
Article 9 Training (assistance available)
Strong time management, communication, and problem-solving skills
