Responsibilities:
- Assist clients with personal care, including bathing, dressing, mobility, and transfers
- Provide companionship and accompany clients to doctor appointments, grocery stores, and pharmacies
- Support active and passive mobility exercises as outlined in the care plan
- Monitor and report any changes in the client’s condition to the Director of Nursing
