Licensed Practical Nurse (LPN) – Night Shift (3x12 Schedule)

Connected Health Care is seeking an experienced and compassionate Licensed Practical Nurse (LPN) to join one of our trusted partner facilities near Wayne, Nebraska. This facility provides both skilled nursing and assisted living services, offering a supportive environment focused on high-quality resident care.

Position Details:

• Shift: Night Shift
• Schedule: 3x12-hour shifts per week
• Setting: Skilled Nursing & Assisted Living

Responsibilities:

• Provide direct patient care in accordance with established care plans
• Administer medications and treatments as prescribed
• Monitor patient conditions and report changes to the care team
• Maintain accurate and timely documentation
• Collaborate with nurses, physicians, and support staff to ensure optimal care

Requirements:

• Active LPN license (Nebraska or compact state)
• Previous experience in skilled nursing or long-term care preferred
• Strong clinical and communication skills
• Ability to work independently during overnight shifts
