Job Summary

New Grad Physical Therapist

TeemaGroup

Full-time

In-Office | Sacramento, CA, United States

Monday to Friday 8AM to 5PM

14-16 patients per day



Responsibilities

  • Provide patient-focused physical therapy assessments and treatment plans.

  • Deliver evidence-based PT interventions to improve functional outcomes.

  • Document patient progress and update care plans in the electronic health record.

  • Collaborate with a multidisciplinary team to coordinate care.

  • Demonstrate accountability for individual targets and performance metrics.

  • Maintain compliance with clinical guidelines and safety protocols.

  • Contribute to and track patient care pipelines to forecast outcomes.

  • Identify opportunities for professional development and skill advancement.

  • Engage patients and families to promote adherence and education.

  • Participate in on-site team meetings and quality improvement initiatives.

  • Support growth targets by identifying potential referrals and community outreach.

  • Demonstrate curiosity and learning mindset through continuous education.


Experience/Qualifications

  • PT licensure with recent graduate eligibility and supervision plan.

  • Completion of entry-level physical therapy assessment and plan development.

  • Evidence-based PT interventions and outcome-driven treatment delivery.

  • Adherence to clinical guidelines, safety, and infection control protocols.

  • Professional development plan with tracked milestones and continuing education.

  • Documentation of patient progress in electronic health records.

  • Multidisciplinary collaboration and care coordination.

  • Participation in care pipelines, forecasting patient outcomes and referrals.

  • Patient and family engagement to promote adherence and education.

  • On-site participation in team meetings and quality improvement initiatives.
