    Job Requirements

    The Case Management Post-Acute Navigator is an integral part of the interdisciplinary team who functions as a liaison between the patient, family, and medical staff. The Case Management Post-Acute Navigator is an advocate for the patient's right to self-determination in his or her care decisions. The Case Management Post-Acute Navigator assesses patients that have been referred for admission using the Post-Acute approved admission criteria.

    Minimum Requirements

    Education

    • Registered Nurse or bachelor's degree in social work, associate degree in health science with a major in respiratory or related field

    Experience

    • 1-3 years of health care experience

    Preferred Requirements

    Preferred Education

    • Bachelor's degree in nursing with a current license to practice as a Registered Nurse in South Carolina or
    • Bachelor's degree in social work and licensed

    Preferred Experience

    • 1-3 years of clinical experience

    Preferred License/Registration/Certifications

    • South Carolina Licensed Registered Nurse
    • South Carolina Licensed Social Worker

    Core Job Responsibilities

    • Assesses prospective patient needs, works closely with referring agencies and physicians, coordinates hospital admissions, and acts a liaison between referring and admitting physicians.
    • Conducts, along with physician leadership support and Utilization Review Committee oversight, initial utilization review activities for the hospital.
    • Must be able to provide and understand the specific communication, developmental and treatment needs of patients, families and visitors ranging in age from early adult to late adult.
    • Provide in services and education for ancillary staff as needed. Assist leadership in reviewing and revising of department policies and procedures.
