Position Summary: This professional position serves as the centralized point of contact for all Multidisciplinary Staffings which include the child, family, natural and community supports, and professionals who collaborate to empower, motivate, and strengthen the family and develop an individualized plan best suited for their needs. This position will ensure staffings occur within established time frames, including all relevant team members, allows for all members to have voice and input, address child safety, permanency, and well-being. Technical/Functional Expectations: 1. Multidisciplinary Team (MDT) Point of Contact-Essential Function: This position is responsible for bringing together a diverse group of individuals for the purpose of allowing better engagement with families and a shared commitment and accountability. This position is responsible for scheduling, coordinating, and facilitating MDTs for all initial placements, changes in physical custody, changes in educational placement, placement decisions involving siblings, and other complex important decisions related to the child. 2. Multidisciplinary Team (MDT) Assessment: Ensure the multidisciplinary team conducts an assessment before formulating a decision. In cases where there is not a unanimous consensus the Department will make a best interest determination and report to the court within established time frames. 3. Transition Plans: Coordinate, track and ensure completion of transition plans for all: LOHC placements, siblings, education within the required time frames. Ensure that all transition plans are documented and uploaded in FSFN. 4. Separated Siblings: Coordinate, schedule and review all separate siblings to assess barriers to placement together and ensure sibling visitation is occurring and contact is being maintained in other ways. 5. On Call Rotation: Participate in the on-call rotation to provide timely placement and service delivery. 6. Customer Service: Work with clients and families to provide support and assist with problem solving. To ensure client satisfaction. 7. Other duties as required: Job performance requires fulfilling other incidental or related duties as assigned, assisting, and training others, and performing duties of higher rated positions from time to time for developmental purposes. Teamwork: Able to cooperate to build consensus; able to assist others to achieve team goals; participate in meetings so they are productive; take on extra work to meet team objectives. 8. Sensitivity: Work effectively with and show sensitivity to cultural differences and various socio-economic backgrounds of others. 9. Ethics: Adhere to and model principles and values of Agency and System of Care by being strength-based, solution-focused, maintains highest level of integrity and ethical standards and work collaboratively with employees, partners, stakeholders, and clients in all interactions. Minimum Qualifications: 1. Bachelors degree in social work, Human Services, Public Administration, or a closely related field required. 2. Minimum of five (2) years experience in child welfare. 3. Child Protection Certification required. 4. Demonstrated knowledge of Florida Child Welfare Statutes, including Chapter 39, as well as applicable administrative codes and regulatory requirements. 5. Valid Drivers license and ability to travel through assigned circuits and other locations as required. Preferred Knowledge, Skills, and Abilities 1. Demonstrated knowledge of Florida's child welfare system, including multidisciplinary team (MDT) processes, permanency planning, placement stabilization, and coordination of services for children and families involved in dependency proceedings. 2. Strong organizational and time management skills, including the ability to coordinate complex case staffing meetings, maintain accurate documentation, track action items, and prepare reports for leadership and system partners. 3. Advanced knowledge of Florida Statute Chapter 39, applicable Florida Administrative Code provisions, and Department of Children and Families policies related to child welfare, permanency, and case planning. 4. Ability to analyze case information, identify service gaps and barriers to permanency, and facilitate collaborative problem-solving to support timely and effective outcomes for children and families. 5. Excellent interpersonal, communication, and facilitation skills with the ability to effectively engage case management staff, caregivers, service providers, legal stakeholders, community partners, and Department representatives in a multidisciplinary team environment. 6. Ability to maintain professionalism, confidentiality, and objectivity while navigating sensitive child welfare cases and balancing competing priorities in a fast-paced environment. 7. Proficiency in Microsoft Office applications and child welfare information systems, with the ability to compile, track, and present data to support case coordination, quality improvement, and performance monitoring efforts. Physical Requirements: 1. Ability to lift and carry up to 20 pounds 2. Frequent sitting and standing 3. Occasional walking, bending, stooping, and crouching 4. Use of hands for grasping, typing, and writing 5. Ability to reach, read, speak, and listen effectively 6.
