    Description

    Position Summary:

    Performs direct and indirect patient care activities under the supervision of licensed personnel. Manages mobilization and ambulation of patients as part of the multidisciplinary care team.

    c. Assists physician and other members of the healthcare team when appropriate.

    d. Answers phone calls, patient call system with a pleasant tone and professional manner.

    f. Demonstrates an awareness of visitation restrictions and special precautions required to enter a patient's room.

    Manages Patient Care

    a. Determine appropriate patients for mobility/ambulation with nursing team.

    b. Coordinates activities with RN Team Leader and Rehab Services team. c. Performs daily mobility/ambulation for patients in a safe manner.

    d. Coordinates and collaborates effectively with other health care disciplines to serve identified patient needs.

    e. Introduces self and explains mobility plan to patient and/or family.

    f. Meets age specific competencies for ages of patients served.

    g. Assists with vital sign measurement/documentation.

    h. Institutes emergency measures for sudden adverse developments in patient's condition and notifies appropriate staff.

    i. Actively participates in falls prevention activities for the neuro unit population.

    j. Performs other clinical duties as needed including meal tray distribution and pickup; responding to patient calls.

    Quality and Safety

    Documents patient care.

    a. Documents appropriately in the medical record including vital signs, intake and output, height and weight, etc.

    b. Documentation is clear, concise and according to hospital policy.

    c. Documentation is completed the same day of service.

    d. Complies with Medicare (CMS), The Joint Commission (TJC), and other regulatory bodies guidelines.

    Observes all safety procedures in accordance with department and UHS policies.

    a. Demonstrates appropriate use of equipment.

    b. Uses appropriate personal protective equipment and standard precautions.

    c. Reports any equipment in need of repair or replacement.

    d. Safeguards against misuse and loss of equipment.

    Educates the patient/family regarding plan for safe mobilization.

    Professional and departmental requirements.

    a. Takes responsibility for CPR and fit testing expiration.

    b. Takes responsibility for Licensure expiration.

    c. Completes mandatory training annually.

    d. Attends and actively participates in department meetings and daily team huddles.
