Provides intensive, goal directed services to support individuals who have a mental health crisis and substance use problems result in the need for crisis stabilization. The team works closely with all Horizons clinical and community partners to provide services at a higher level of intensity or frequency with the goal of community tenure by the consumer.

Mobile Crisis Case Manager - Horizons - PRN

Minimum Qualifications:
Required Education and Experience
Bachelor's degree, preferably in Social Work, Psychology, or some related field
Associate's degree with an additional 2 or more years' experience working with educating and teaching clients about self-care, improving daily living activities
Total combined years of schooling and experience (working with educating and teaching clients about self-care, improving daily living activities) to equal 4 or more years
Required License/Certifications/Registrations
CPST Case Management training, KHS training, pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens.
Completion of Adult Case Management training within 60 days of employment.
Valid driver's license, insurance, and a dependable vehicle if position requires driving.
Preferred Qualifications:
Preferred Education and Experience
Bachelor's degree in Social Work, Sociology, Psychology, or Special Education (with specialized training in human development and family dynamics).
Three to five years' professional experience in mental health services, crisis response, or a related field.
Two years or more of experience working with educating and teaching clients about self-care, improving daily living activities.
Preferred License/Certifications/Registrations
Knowledge, Skills, and Abilities:
Familiarity with community mental health services.
Strong organizational skills
Intermediate proficiency with Microsoft Office
