As a Mid-Level Practitioner, you will play an integral role in delivering high-quality patient care across our medical specialties while working alongside physicians, nurses, counselors, and administrative staff dedicated to improving patient outcomes.

Position Summary

The Mid-Level Practitioner functions as an essential member of the healthcare team by providing direct patient care, evaluating and treating patients, developing individualized treatment plans, and ensuring all services meet established clinical, regulatory, and organizational standards.

Essential Responsibilities

• Perform comprehensive patient evaluations and medical histories.
• Conduct physical examinations.
• Diagnose and treat acute and chronic medical conditions within scope of practice.
• Develop and implement individualized treatment plans.
• Order, interpret, and review laboratory and diagnostic testing.
• Prescribe medications and treatments in accordance with Ohio law and DEA regulations.
• Document patient encounters accurately and timely within the electronic medical record.
• Educate patients regarding diagnoses, treatment plans, medications, and preventive care.
• Collaborate with physicians and interdisciplinary team members to coordinate patient care.
• Ensure compliance with clinical protocols, regulatory requirements, and quality standards.
• Participate in quality improvement initiatives and continuing education.

Qualifications

Required

• Master's or Doctoral degree as a Nurse Practitioner or Physician Assistant from an accredited program.
• Current Ohio Nurse Practitioner or Physician Assistant license.
• National certification (AANP, ANCC, or NCCPA).
• Current Ohio DEA registration.
• Active National Provider Identifier (NPI).
• Excellent communication and interpersonal skills.
• Strong clinical judgment and critical thinking abilities.
• Ability to work effectively in a collaborative healthcare environment.

Preferred

• Experience in Addiction Medicine, Primary Care, Family Practice, Internal Medicine, Emergency Medicine, or Women's Health.
• Experience with electronic medical records (EMR/EHR).
