    JOB SUMMARY Coordinates, oversees and directs all Care Management staff services, promotes optimal Care Management services through assessments, clinical interventions, and advocacy; directs the complete referral/ admissions for LTC to ensure appropriate candidates, services and revenues. ESSENTIAL FUNCTIONS Coordinates, oversees, directs LTC Care Management Staff By conducting regular staff meetings, arranging individual employee support services, providing education and training, managing schedules, managing the proper use of PTO time, scheduling and mentoring of Graduate Interns, conducting evaluation of programs and the quality of work at both LTC sites Coordinates, oversees, directs LTC Care Management services and interventions By coordinating and reviewing services provided, performing audits, providing clinical consultation to department staff and ensuring resident needs/outcomes within regulatory/professional guidelines and standards. Coordinates and works toward financial objectives and strategic planning By preparing/managing departmental budgets, communicating budgetary information to all staff, recommending system improvements, coordinating efforts with fund raising departments, maximizing efficiency and productivity, overseeing and coordinating Specific Purpose Restricted Funds. Serves as an Advocate/Representative of LTC Care Management Staff and Serves as a strong Interdisciplinary Team member By attending a variety of internal and community meetings, maintaining contact with interdisciplinary leaders, encouraging open lines of communication and problem solving initiatives between departments and with the LTC management teams at both facilities as well as with the organizations administrative leaders, attending interdisciplinary GS functions/events. Coordinating All LTC Referrals and Admissions By securing and screening potential applicants, securing appropriate information regarding care needs, guiding the admissions committee through the evaluation of applicants, matching needs with program services, managing waiting list activities, communicating care needs to interdisciplinary team representatives prior to admissions, coordinating all admission day details and activities. EVALUATE RESIDENTS PSYCHOSOCIAL ABILITIES AND NEEDS By Systems Based clinical assessments including but not limited to PhQ-9, psychosocial assessments, depression screenings, suicide risk assessments, drug/alcohol screenings, behavioral assessments, screening for potential discharge planning needs and collateral interviews. DEVELOPS AND PROVIDES THERAPEUTIC INTERVENTION PLAN ACCORDING TO NEEDS By individual/group psychosocial counseling, promoting resident empowerment through the development of life management skills, developing and implementing behavior management plans, providing crisis intervention/conflict resolution and/or problem solving, acting as liaison with other mental health practitioners, providing support with goal planning and implementation. DEVELOPMENT AND IMPLEMENTATION OF STAFF TRAINING By formal presentation of individual and group training sessions, informal staff support, development and distribution of written material, staff consultations in addition to providing both general and specific educational resources COORDINATION OF SERVICES FOR RESIDENTS By building functional partnerships with community based resources for the purpose of exploring and securing appropriate services to meet individual resident needs. Resources may include but are not limited to: mental health supports, social groups, education services, employment opportunities, financial management, etc. ADVOCATES FOR THE RIGHTS AND NEEDS OF DISABLED RESIDENTS By advocating on behalf of each/all residents both within the organization and throughout the community, coordinates and participates in resident and family council groups. COORDINATES RESIDENT DRIVEN DISCHARGE PLANNING By implementation and coordination of Community Transition Program for each resident as appropriate. Tasks may include but are not limited to coordination of interdisciplinary team assessments/treatment plans, community referrals, support with skill building, and the procurement/establishment of equipment and services, etc. MAINTAINS DOCUMENTATION By preparing written documentation of clinical assessments and interventions, making functional and timely notations in the medical record, maintaining accurate departmental files, providing required financial documentation to multiple state/federal offices of benefit management and ensuring the satisfaction of all regulatory standards relative to documentation within the social work scope of service. COORDINATION OF COMMUNITY ACCESS PROGRAM By developing and implementing new programs for residents, managing efficiency and timeliness of services, managing LTC Care Management Restricted Fund, directing the Community Access Committee members through tasks associated with the Community Access Program. QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
