Mental Health Therapist - LCSW

HJMC is a Federally Qualified Health Center who provides a range of medical, dental, and behavioral health services for the entire family. Mental Health Therapists at HJMC offer brief counseling and psychotherapy to adults, children, adolescents and their families in order to improve psychosocial functioning. The therapist part of a multidisciplinary team that work together with medical providers to offer services through an integrated care model. This is an exempt position and the incumbent will be expected to work a minimal of 40 hours a week with evening hours.

Essential Duties and Responsibilities:

• Provide individual, family and group mental health counseling for patients.
• Deliver individual and small group counseling services using evidence-based or best practice models of care.
• Complete required documentation promptly and efficiently, according to organizational policy.
• Participate with the HJMC interdisciplinary team to provide mental health care recommendations for patients and/or multi-disciplinary team meetings with outside agencies.
• Consult with primary care provider, psychiatrist, substance abuse counselor, care coordinator, and other mental health professionals as needed.
• Make appropriate referrals as needed based on clinical judgment.
• Maintain professional behavior and relationships with HJMC staff and referral resources.
• Participate in activities that promote professional development and use clinical/ educational research to enhance therapy services.
• Identify psychological, emotional or behavioral issues and diagnose disorders, using information obtained from interviews, tests, records, consultations with primary care providers and reference materials.
• Support medication therapy prescribed by PCP.
• Attend staff meetings and other meeting as required.
• Perform general administrative/office duties as required.
• Fill out and maintain client-related paperwork.
• Provide crisis intervention, emotional support, resources and referrals as needed.
• Screen and assess patients for common mental health and substance abuse disorders.
• Facilitate patient engagement and follow-up care.
• Utilize registry and tracking systems to collect and collate data.
• Document patient progress and treatment recommendations in EHR and other required systems.
• Facilitate treatment plan changes for patients who may need more intensive or more specialized mental health care.
• Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
• Support the mental and physical health care of patients on an assigned patient caseload.
• Closely coordinate care with the patient’s medical provider and, when appropriate, other mental health providers.
• Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g. problem-solving treatment or behavioral activation) as clinically indicated.

Position Requirements

Qualification: To perform this job successfully, an individual must be able to perform each essential duty satisfactory.

• Licensed Clinical Social Worker or Licensed Professional Counselor and with up-to-date Delaware Licensure Required
• Excellent working knowledge of behavioral medicine and evidence-based behavioral treatments for chronic disease, addiction, and mental health conditions
• Ability to work comfortably and effectively within an interdisciplinary primary care team
• Possess strong written and verbal communication skills
• Ability to design and implement clinical pathways and protocols for treatment of selected chronic conditions in and individual and group modality
