Montare Behavioral Health is currently hiring a Case Manager for our Mental Health and Substance Use Outpatient Treatment Program!The Case Manager provides direct support and care coordination for clients receiving outpatient treatment for mental health and substance use disorder diagnoses. The Case Manager assists clients with treatment engagement, discharge planning, connecting to community resources, life skills and goal development, while supporting a safe, structured, and therapeutic treatment environment.This role works collaboratively with a multidisciplinary treatment team to help clients build stability, increase independence, and successfully transition to lower levels of care and community living.Essential Duties:Support clients with scheduling appointments, transportation coordination, housing and employment resources, and other psychosocial needsCoordinate care and maintain regular communication with clients, families, referral sources, outside providers, and community agenciesAssist clients with developing and progressing toward individualized treatment goalsSupport clients in developing independent living skills, healthy coping strategies, and community reintegration skillsFacilitate discharge planning, aftercare coordination, and linkage to community resourcesMaintain accurate, timely, and compliant clinical documentation within the electronic medical recordParticipate in multidisciplinary treatment team meetings and provide client updatesProvide psychoeducation, support, advocacy, and encouragement to clients Assist in crisis intervention and de-escalation when necessaryFacilitate groups as neededCollaborate with the treatment team including therapists, medical providers, nursing and direct care staff Participate in ongoing trainings, supervision, and quality improvement initiatives Qualifications:One year full-time Case Management experience working directly with clients with mental health and substance use disorder diagnoses, preferably in a treatment setting.CADC (or equivalent substance abuse counseling certificate); or RADT with all education and supervision hour requirements for counseling certification close to being met, required. Knowledge of and belief in the principles of client centered treatment. Demonstrated ability to work independently and as a team player. Excellent oral and written communication skills.Demonstrate professionalism and maintain strong boundaries with all clients and staff.
