Position Purpose:

The Mental Health Rehabilitation Specialist provides intensive, field-based rehabilitative and supportive services to adults experiencing serious mental illness, homelessness, and co-occurring conditions enrolled in a Full Service Partnership (FSP) program at Jwch Institute Inc. Working as part of a multidisciplinary team, the Rehabilitation Specialist supports clients in achieving stability, independent living, and recovery through a "whatever it takes" approach consistent with Department of Mental Health (DMH) and Medi-Cal requirements. Services are delivered primarily in the field and include mental health rehabilitation, targeted case management, and crisis intervention

Principal Responsibilities:

  • Conduct field-based work (i.e. County Jail and community based)
  • Conduct screenings for all service requests that come in in a timely manner (as defined by DMH Access to Care standards)
  • Work collaboratively with a multidisciplinary team to develop a comprehensive care plan and monitor on an ongoing basis. Communicate client progress, needs, and risks to the team
  • Provide rehabilitation and skill-building services to support independent living, symptom reduction and community engagement
  • Provide group rehabilitation service to clients enrolled in the program
  • Provide ongoing case managements services including linkage and referral to both internal programs and external resources
  • Accompany clients to appointments (medical, psychiatric, benefits, housing)
  • Support clients in accessing public benefits (SSI, General Relief, Medi-Cal)
  • Assist clients with housing stabilization and retention
  • Provide crisis intervention and de-escalation in community settings
  • Maintain productivity requirements to meet contract compliance.
  • Complete all charting as required by funding sources, including assessments, progress notes, care plans, and billing documentation on timely basis.
  • Participate in structured and "as-needed" case conferences
  • Collect necessary data for reporting purposes, as requested
  • Participate in agency and community meetings.
  • Coordinate outreach activities in and outside of clinic
  • Adhere to all organizational safety policies, procedures, and regulatory requirements to ensure a safe environment for patients, staff, and visitors.
  • Participate in required safety and compliance trainings and apply learned practices in daily work activities.
  • Identify, report, and help mitigate unsafe conditions, incidents, or hazards in the workplace.
  • Use equipment, tools, and supplies safely and responsibly in accordance with organizational and regulatory standards.
  • Maintain awareness of infection control, occupational health, and emergency preparedness procedures appropriate to the role.
  • Perform other duties as assigned

Requirements:

  • Bachelor's degree and four (4) years of experience in a mental health setting required. Two (2) years of graduate education may be substituted for the required experience, or two (2) years of post-Associate degree clinical experience may be substituted for the educational requirement.
  • Two years of experience working in the Skid Row community or similar high-need, unhoused, low income, underserved environments
  • Ability to work in high-acuity, field-based environments
  • Knowledge of trauma-informed care, harm reduction, and Housing First principles
  • Strong organizational, time management and documentation skills
  • Cultural humility and ability to work with diverse populations
  • Experience with Electronic Health Records (EHR) systems
  • Valid California driver's license
  • Experience working in a DMH funded program
  • Bilingual (English/Spanish) ability preferred
