Mental Health Agency now hiring Experienced PT Qualified Professionals with extensive mental health experience to work in office/ community to complete PCP/Crisis Plan and updates.
Will complete intake paperwork, PCP/ Crisis Plans and Treatment Team meetings.
Experienced, detail-oriented, driven, professional who is great with paperwork and a community liaison and can adhere to the turnaround time.
Must have own transportation.
Experienced with note review and supervision, and adhering to company deadlines and policies.
Respond if experienced with proven results.
Qualified Professional Duties
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following: Other duties may be assigned.
Completion of the intake/referral form with the individuals being served.
Provide input into person-centered plan modifications, as needed
Coordinate activities within the established plan; facilitate plan meeting;
Assist with referral linkage to other services and community resources
Minimize the negative effects of psychiatric symptoms or substance dependence that interfere with the person supported daily living and personal development
Work closely with other Para-professional/Certified Peer Support Professionals and clinical/professional staff to maintain communication and provide feedback, standardize procedures within the agency.
Facilitate relationships and serve as a link between the agency, parents, guardians, natural supports, other service providers, and the community;
Facilitate and/or participate in treatment team meetings (if indicated)
Ensure that all documentation is accurate and reflective of services and completed on time as defined by the agency's policy and state guidelines
Assist with the development and design of training modules for staff within the agency and administer staff training, as needed.
