Pride Health is hiring a Mental Health & Psychiatric Registered Nurse to support our client's healthcare facility in Bronx, NY.

This role is ideal for a nurse with strong case management skills and experience working with individuals who have mental health or psychiatric needs. The RN will play a key role in supporting residents' overall health and wellness through care coordination, medical follow-up, wellness monitoring, and documentation. This position does not include medication administration or medication management responsibilities.

Job Details:
Schedule: 
Monday through Friday (10:00 AM to 6:00 PM) or (11:00 AM to 7:00 PM)
Location: Bronx, NY
Job Type: Contract
Contract Length: 13 Weeks, with possibility of extension
Pay Range: $43hr/hr. - 50/hr.
*Pay offered is based on experience, expertise, credentialing, and education.

About The Program Supported
This position is based within a supportive housing program serving approximately 86 residents. Residents live independently in their own apartments and are generally able-bodied. While some individuals may have mental health diagnoses or other challenges, the population does not require direct support professional (DSP) services.

Duties:
  • Coordinate and assist residents with medical appointments and follow-up care
  • Provide wound care and routine nursing support as needed
  • Conduct wellness assessments and routine health checks
  • Follow up with residents after hospital visits and assist with care transitions
  • Maintain accurate and timely documentation of all resident interactions, medical follow-ups, and related services
  • Utilize the AWARDS electronic health record/charting system
  • Collaborate with program leadership and community healthcare providers to support resident well-being
Requirements:
  • Active New York State Registered Nurse (RN) License and Registration
  • BLS/CPR Certification through the American Heart Association (AHA)
  • Professional Malpractice Insurance
  • Associate's Degree in Nursing or higher
  • 2 years of experience working with mental health and/or psychiatric populations
  • Strong nursing case management and care coordination skills.
