Sunrise Services, INC. is seeking a Mental Health Professional to join our 16-bed Enhanced Services Facility!
Offering a $2,000 sign on bonus!
Duties & Responsibilities:
- Assess resident need, risk factors and barriers to stable ESF / community placement at referral. Identify and record baseline presentation of 'challenging behaviors' at referral and track progress during ESF placement.
- Identify existing services and supports (informal and formal) involved with resident. Seek to identify most appropriate role and input to compliment and extend existing service provision in the interest of facilitating a successful ESF placement
- Work in conjunction with existing mental health providers when present - seek to clarify and agree complimentary role to meet residents' needs and facilitate successful ESF placement
- Participate in the creation of effective resident specific Person Centered Service Plans (PCSP) to be utilized by residents and PCSP Team members. Monitor use of the plans and provide trainings and consultation as needed to ensure effective use of the plan. Update and modify plans as needed
- Coordinate services with community agencies such as DSHS, doctors, hospital staff, discharge planners, other MH Providers, courts, Day Services, Employment Support agencies, leisure time activities, etc.
Seek consultation and feedback from residents, family members, agency staff and residential providers.
- Record, track and monitor presentation of challenging behaviors at referral, placement, 30,60, and 90 days as needed.
- Provide clinical assessment and support to residents as specified in each resident's person centered service plan.
Schedule:
Part time or Full time
20-40 hours per week. The facility is open 24/7.
Education:
- Master's Degree or further advanced degree in counseling or one of the social sciences OR Psychiatric Nurse
Experience: At least two years of experience in supervised direct treatment of persons with mental illness and/or developmental disabilities, or a Mental Health Professional (MHP), or a professional with significant career related experience working towards hours of completion for MHP status.
Licensure/Certification: Mental Health Professional (MHP), or a professional with significant career related experience working towards hours of completion for MHP status.
National Provider Identifier (NPI): Must be active with Sunrise reflected as agency designation.
Other Considerations:
- DSHS background clearance required.
- Current valid Washington State driver's license, acceptable driving record, and proof of insurance.
- Skill using Microsoft Office products including Word and Outlook.
- Strong verbal and written communication skills.
- Flexible. Demonstrated ability to exercise sound judgment in challenging dynamic situations including behavioral issues.
- Skill in reading, synthesizing, and acting upon multifaceted information.
- Ability to write effective positive behavior support plans specific to resident need
- Ability to train and coach caregivers in the implementation of positive behavior support and de-escalation.
Compensation:
- $28 - $42 per hour
- Wages are determined by applicable industry experience, education, and certifications/licensure.
