The Mental Health Professional III role will deliver exceptional psychiatric treatment to patients and their loved ones. The position prioritizes the satisfaction of their patients by offering patient-centered and high-quality care. The responsibilities include evaluations, psycho-education, group activities, planning for treatment, mapping out discharge, and providing therapeutic counseling help. They work to produce positive outcomes for their patients by using verified techniques in one-on-one, family, and group counseling sessions.
What will you do in this role:
SPECIFIC ELEMENTS AND ESSENTIAL FUNCTIONS:
TheSocial Worker will exhibit excellent customer relations to patients, visitors, physicians, and co-workers as well asshowscourtesy, compassion and respect to all individuals.
1. Responsible for providing social work services to patients and their families, as a member of a multidisciplinary team.
2. Social work interventions are focused on assessment, crisis stabilization,psycho-therapy, and disposition planning.
3. At times, the social worker may provide service to different patient age groups, based on training and knowledge.
4. Screen admissions for discharge needs within one-two (1-2) business days.
5. Track andmonitoravoidable (alternative care)days, andprovide your supervisor with this information daily.
6. Performs continuous assessments andevaluations,to ensurepatientis progressing towards desired outcomes.
7. Assesses and responds to patient/family needs by coordinating efforts of other team members.
8.Identifiesand resolves barriers that hinder effective patient care.
9. Collaborates and consults with physicians onpatient'sprogress and discharge planning needs.
10. Communicateseffectiveand professionally with peers, supervisors, ancillarydepartmentsand all other external/internal customers.
11. Coordinates the discharge planning process ensuring involvement of all members of the healthcare team.
12.Counselswith patients and family members in decision making and in meetingpsychosocialneeds of the patient.
13. Maintains knowledge of resources and facilities available to patients and family members.
14. Collaborates with personnel at other facilities to coordinate smooth and effective patient transfers and transitions.
15. Compiles, evaluates, andreportsstatistics to members of the team andutilizesthe information tofacilitateprocess improvement activities.
16. Analyzes pertinent data, communicates findings, documents information accurately.
17. Provides age and culturally appropriate care.
18. Orients and mentorsnew staffmembers.
19.FollowsStandard Precautions using personal protective equipment as required.
20. Collaborates with RNCaseManager to coordinate smooth and effective patient transfers and transitions.
21. Takes an active role in detention order hearings on the behavioral health unit.
EDUCATION:
Master’s Degree from an accredited social work program orMaster’s Degreein a related fieldthatprovidedappropriate backgroundand experience. Education should be clinically focusedwith study inhuman life span development and psychopathology.
LICENSURE/CERTIFICATIONS:
Current VA License as a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC)
BLS for Healthcare Providers through the AHA or CPR through the American Red Cross
Crisis Prevention Institute certificationisrequiredwithin30 daysof hire
EXPERIENCE:
At least the equivalent of 2 years of experience, which can include a graduate level practicum (20 hours per week for 2 semesters or40 hoursper week for 3 months).Experience must be focused in the area of inpatient hospitalization, partial hospitalization, crisis prevention, or outpatient practice.
