    Job Summary:

    The LMHC or LCSW will provide in-person professional counseling services, including assessment, counseling, case management, and care coordination to a diverse patient population of individuals and families. The therapist will collaborate with the interdisciplinary team to develop and implement care plans that address the patient’s psychosocial needs, promote well-being, and support overall treatment goals.


    Key Responsibilities:

    • Provide initial intake, diagnostic evaluations, treatment plans and progress notes.
    • Complete progress notes within 24 hours of time of service.
    • Maintain client’s charts and keep complete and correct documentation
    • Case management including care coordination, collaborating with referral sources and connecting clients to care.
    • Complete timesheet daily reflecting the billable hours worked

    Qualifications:

    • Masters degree in Mental Health Counseling or Social Work
    • Minimum of 2–3 years of experience in mental health services, case management, or program coordination.
    • Experience working in clinical or community mental health settings preferred.
    • Demonstrated ability to coordinate multidisciplinary care and support for individuals with mental health needs.
    • Ability to work with diverse populations and manage sensitive and confidential situations with professionalism and empathy.
    • Knowledge of HIPAA and other regulations related to mental health care.
    • Proficiency with electronic health records (EHR) and standard office software (e.g., Microsoft Office, Google Workspace).

    Physical Requirements:

    This position primarily operates in an office environment. The role requires the ability to sit for extended periods, use a computer and other standard office equipment, and occasionally lift or move items up to 10–15 pounds. Walking, bending, and standing may be required.
