    Responsibilities:

    • Provides and supervises the care and education of patients and families with memory disorders.
    • Leads the Memory Disorders Center team to develop and implement programs such as inpatient and outpatient programs, support groups, community education, memory screenings, wellness retreat and special events, and research involvement.
    • Participates in quality improvement and is an expert resource for pharmacists, occupational therapists, psychologists, dietitians, nursing, case management, physicians, etc. and navigates the transition between inpatient and outpatient.
    • Actively promotes the Memory Disorders Center in the community to increase awareness and referrals

    Required Education and Experience:

    • 5 years of experience in Social Work and Memory Disorder Care.
    • Masters Degree in social work
    • BLS (Basic Life Support)

    Preferred Education and Experience:

    • Licensed Clinical Social Worker with either Certified Montessori Dementia Care Professional (CMDCP) or Certified Dementia Practitioner (CDP) certification.
