    Description

    We are looking for a Memory Care Director (LPN) to join our team!

    The Memory Care Director will be responsible for ensuring the highest quality of care and support for residents living with Alzheimer's disease, dementia, or other memory-related conditions. This individual will lead a team of caregivers and staff, develop and implement memory care programs, and foster a nurturing environment that promotes the well-being and dignity of our residents.

    Responsibilities:

    • Oversee all aspects of the Memory Care department
    • Provide visionary leadership and strategic direction for the Memory Care community, ensuring adherence to best practices and regulatory requirements.
    • Conduct regular performance evaluations, provide constructive feedback, and implement professional development opportunities for staff.
    • Assess the individual needs and preferences of residents living with memory-related conditions, developing personalized care plans to promote independence, dignity, and quality of life.
    • Coordinate and oversee the delivery of care services, including assistance with activities of daily living, medication management, and specialized memory care programs.
    • Monitor residents' health and well-being, collaborate with healthcare professionals, and respond promptly to changes in condition or medical emergencies.
    • Develop and implement innovative memory care programs and activities that stimulate cognitive function, promote social engagement, and enhance overall quality of life for residents.
    • Build positive relationships with residents' families, providing regular updates, offering support, and addressing any concerns or questions in a timely and compassionate manner.

    Requirements:

    • Must hold a valid LPN certification in the state of Virginia.
    • Strong knowledge of Alzheimer's disease, dementia, and other memory-related conditions, along with proven experience in developing and implementing effective memory care programs.
    • Minimum of 2-3 years of experience in Memory Care or related healthcare setting.
    • Previous management experience and ability to lead the team by example and provide care on the floor as needed
    • Able to pass a background check and company-policy drug screen
