    The Springs at Clackamas Woods is looking for an experienced Memory Care Coordinator to lead and support our Memory Care program. This role is ideal for someone who combines healthcare experience with a genuine understanding of dementia care and a passion for supporting residents, families, and the caregivers who provide their daily care.

    As our Memory Care Coordinator, you’ll play an important role in creating a consistent, person-centered experience for our residents while providing leadership, guidance, and support to the Health Services team.

    Responsibilities:

    • Oversee our Memory Care program and help ensure residents receive consistent, person-centered care.
    • Develop, review, and update service plans in partnership with residents, families, and the care team.
    • Train, coach, and support caregivers and Med Techs in Memory Care practices and approaches.
    • Build relationships with residents and families and communicate about resident needs, preferences, and changes.
    • Ensure care practices, documentation, and daily operations meet applicable Memory Care standards and regulations.
    • Partner with community leadership to support team development, resolve concerns, and continuously improve the resident experience.
    • Participate in the on-call rotation to respond to open shifts.

    Qualifications

    • Previous experience working with older adults living with dementia or other cognitive changes.
    • Experience in senior living or healthcare; medication administration experience preferred.
    • 3–5 years of healthcare experience.
    • Licensed Administrator required.
    • Knowledge of Memory Care regulations and care standards.
    • Strong communication, interpersonal, organizational, and problem-solving skills.
    • Ability to effectively communicate with residents, families, employees, managers, and healthcare professionals.
    • Strong computer skills, including electronic records, email, word processing, and spreadsheets.
    • Ability to read and interpret charting standards, physician orders, regulations, and other written instructions.
    • Ability to successfully complete a criminal background check and pre-employment drug screen.
    • Must be at least 18 years of age.
    • Excellent attendance and flexibility to support the needs of the community.
