Family Life Care is seeking a dedicated and compassionate Medical Social Worker (MSW) to join our home health team. The Medical Social Worker provides professional social-work services to patients and their families in the comfort of their homes.This position evaluates the social, emotional, financial, environmental, and family circumstances that may affect a patient's health, safety, treatment, or recovery. The Medical Social Worker works closely with physicians, nurses, therapists, and caregivers to address barriers to care and connect patients with appropriate resources.Family Life Care proudly serves individuals throughout the community, from children to senior citizens. We offer various assignments and work with team members to develop schedules that support both patient needs and work-life balance.Essential Duties and ResponsibilitiesPatient assessment, and coordination of care with other disciplines, physicians and care managers.Treatment of home care patients in accordance with the plan of treatment and with company policies and procedures.Provides services in a pleasant, professional manner to maximize positive outcomes and patient satisfaction.Ensure that all clinical assessments, care plans, starting and resumption of care are completed with agency guidelines. Must be deadline driven.Responsible for compliance with all state and federal, laws, regulations, policies, and processes relevant to home health.Required QualificationsMaster of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education is required.Minimum of one year of social-work experience in a healthcare setting.Current Florida social-work license.Current CPR certification.Valid driver's license, reliable transportation, and current automobile insurance.Ability to successfully complete a Level II background screening and all other required pre-employment screenings.Ability to travel to patients' homes within the assigned service area.Ability to complete documentation accurately and within established deadlines.Preferred QualificationsPrevious medical social-work or home health experience.Knowledge of Medicare, Medicaid, managed-care, and community-based assistance programs.Experience working with children, adults, senior citizens, veterans, individuals with disabilities, and their families.Knowledge, Skills, and AbilitiesCompassionate, approachable, patient-centered, and supportive professional manner.Strong psychosocial assessment, counseling, crisis-intervention, and problem-solving skills.Knowledge of community resources, long-term care services, patient-advocacy programs, and public-benefit programs.Strong interpersonal, verbal, and written communication skills.Ability to work independently in patients' homes while collaborating effectively with an interdisciplinary care team.Strong organizational, time-management, and documentation skills.Ability to manage multiple patients, referrals, and deadlines.Sound professional judgment and respect for patient rights, dignity, preferences, and cultural differences.Comfortable using electronic medical records, mobile applications, and other agency technology.Willingness to go the extra mile to support patients, families, and coworkers.Physical and Work RequirementsAbility to travel regularly throughout the assigned service area.Ability to enter and work safely in a variety of home environments.Ability to sit, stand, walk, bend, and carry the equipment or materials necessary to perform assigned duties.Ability to communicate effectively with caregivers, healthcare professionals, patients, and their families.Equal Employment OpportunityFamily Life Care is an equal opportunity employer.
