About the Role
Make a meaningful difference in the lives of patients and families by providing compassionate psychosocial care and support during some of life's most challenging moments. As a Medical Social Worker with Optimal Care, you'll help individuals navigate the emotional, social, and practical challenges of illness while ensuring their unique needs, values, and goals remain at the center of their care. Whether helping patients overcome barriers to recovery and maintain independence through Home Health or providing comfort, guidance, and emotional support during the end-of-life journey through Hospice, you'll play an essential role in promoting dignity, quality of life, and overall well-being.
This role is ideal for a licensed social worker who is passionate about whole-person care, building meaningful relationships, and helping patients and families navigate complex healthcare decisions. You'll conduct comprehensive psychosocial assessments, provide counseling and crisis intervention, coordinate community resources, and support individualized care plans that address each patient's emotional, social, and practical needs. Working closely with nurses, physicians, therapists, caregivers, and other members of the interdisciplinary care team, you'll help patients and families make informed decisions, overcome challenges, and access the resources they need. Whether your passion lies in supporting recovery and independence or guiding families through life's final chapter, you'll have the opportunity to make a lasting impact through compassionate, patient-centered care.
