Description

The Hospice Medical Records Specialist will assemble and distribute medical records according to established procedures. This position is responsible for the successful coordination of all medical record activity pre- and post-service to ensure a smooth transition of information.

Hours: Full-time Monday - Friday | 8:00am-5:00pm | This position will primarily work in-office and may be eligible to work a hybrid schedule after a successful training period to work remote 1 day a week.

A few must-haves for this position:

  • High School Diploma
  • Two (2) years office experience required, preferably in the medical field.

Some functions of what you will do: 

  • Scans medical records into each patient’s electronic medical chart.
  • Tracks incoming and outgoing medical documents on a daily basis.
  • Prepares and sends verbal orders and other documents to physicians and other providers, as needed. 
  • Conducts regular audits of medical records and follows up on needed corrections.
  • Monitors and follows up on incoming faxes.
  • Assists with updating, ordering and printing forms/documents, as needed. 
  • Coordinates with contracted outside agencies for patient needs.
  • Acts as a contact telephone liaison person interfacing with referral sources, outside agencies, health professionals and field staff, as needed. 
  • Receives incoming calls, providing patients/clients and referral sources with basic information to assist in accessing appropriate services, as needed.
  • Communicates with patient/client and family regarding arrangements for the initiation of care/services, as needed.
