    Medical Records & Intake Assistant

    JOB SUMMARY

    The Medical Record & Intake Assistant is responsible for medical records, data entry, maintaining patient records, preparing for meetings, answering & directing telephone calls to the appropriate staff, and entering/coordinating patient intake. They are responsible for managing workflow in Home Care Home Base to support operations of the Agency. This position also requires establishing and maintaining positive relationships with all related entities (internal staff, clinical field employees, contract staff, patients/families, referral sources, etc), responding to staff requests/concerns and maintaining information to assist in the coordination of patient services and staff availability. They are responsible for all aspects of the patient intake process and maintaining and protecting organization clinical records

    DUTIES & RESPONSIBILITIES

    1. Maintains confidentiality of patient information.

    2. Assists the Intake department with entering and coordinating new patient referrals.

    3. Maintains accurate up to date medical records in accordance with regulations of Medicare and

    the Policy and Procedure Manual.

    4. Communicates effectively on the telephone with patients, families and staff.

    5. Answers incoming calls and forwards to appropriate staff person or takes a message if the
    person is not available.

    6. Welcomes and assists all guests.

    7. Manages incoming, outgoing and interoffice mail.

    8. Performs typing, faxing and copying tasks as requested for various staff persons.

    9. Inputs data into computer for billing purposes.

    10. Orders and maintains accurate records of medical equipment and supplies.

    11. Responsible for informing staff of any changes, updates, etc., regarding admissions; discharges,

    visits, schedules; and clinical meetings as directed by the Program Director, Director of Clinical
    Services and/or Administrator.

    12. Tracks admission, discharge, IDTs, certification and re-certification dates on all patients.

    13. Responsible for assisting with audits of patient information.

    14. Maintains accurate mailing lists of employees; patients; volunteers, Medical Directors,
    physicians and nursing facilities and forwards any changes to the Corporate Office.

    15. Protects the confidentiality of patient and organization information through effective controls

    and direct supervision of clinical records.

    16. Forwards copies of clinical records to authorized users according to policy.

    17. Monitors aged unsigned physician orders and submit second and third requests for signed

    orders to overdue physicians, calls or visit the offices of seriously overdue physicians.

    18. Completes an administrative record audit following patient discharge and forwards abnormal

    results to the Clinical Director or their designee for clinical audit.

    19. ensure accurate and prompt filing and retrieval of clinical records.

    20. Performs other necessary functions/duties as assigned by the Executive Director.

    Home Health Aide (CNA)

    JOB SUMMARY
    The Certified Nursing Assistant (CNA) is a paraprofessional member of the home care team who works
    under the supervision of a registered nurse or therapist and performs various personal care services as
    necessary to meet the patient's needs. The home health aide is responsible for observing patients,
    reporting these observations and documenting observations and care performed. The home health aide
    will be assigned in a manner that promotes quality, continuity and safety of a patient's care, and only after
    successful completion of a competency evaluation program. CNA, is also a part of the interdisciplinary
    team.

    DUTIES & RESPONSIBILITIES
    Responsibilities of the certified nursing assistant include, but are not limited to, the following:
    1. Providing personal care including:
    A. Baths
    B. Back rubs
    C. Oral hygiene
    D. Shampoos
    E. Changing bed linen
    F. Assisting patients with dressing and undressing
    G. Nail & Skin care to prevent breakdown
    H. Assisting the patient with toileting activities
    I. Keeping patient's living area clean and orderly, as appropriate
    2. Planning and preparing nutritious meals.
    3. Assisting in feeding the patient, if necessary.
    4. Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure
    when ordered (with appropriate completed/demonstrated skills competency).
    5. Assisting in ambulation and exercise according to the plan of care.
