Job Title: Medical Records Clerk
Location: New York, NY
Job Type: Contract
Duration: 8 weeks
Shift: Day | 9:00 AM–5:00 PM
Schedule: 7 hours/day, 5 days/week, 35 hours/week

Position Overview
We are seeking a Medical Records Clerk to accurately prepare, scan, index, file, and maintain medical records while ensuring confidentiality and compliance with HIPAA requirements.

Required Qualifications

  • High School Diploma or GED.

  • Minimum 1 year of experience working directly with paper medical records.

  • Minimum typing speed of 40 WPM with high accuracy.

  • Proficiency in Microsoft Excel, including data entry, sorting, and filtering.

  • Ability to identify, classify, assemble, and accurately file medical record documents.

  • Strong data-entry, attention-to-detail, and phone-handling skills.

  • Ability to maintain strict patient information confidentiality and comply with HIPAA regulations.

  • Ability to lift record boxes up to 25 lbs and stand, bend, and reach for extended periods.

Preferred Qualifications

  • Prior hospital experience in Health Information Management (HIM) or document imaging.

  • Familiarity with Epic EHR.

  • Knowledge of medical terminology.

Responsibilities

  • Prepare, scan, and index paper medical records into the electronic health record system.

  • Assemble, sort, and batch medical record documents according to established guidelines.

  • Identify and classify medical record documents, including histories and physicals, operative reports, progress notes, nursing documentation, orders, consents, lab results, imaging reports, and discharge summaries.

  • Scan documents and verify image quality, orientation, page counts, and completeness.

  • Index documents to the correct patient, encounter, date of service, and document type.

  • Review scanned records and correct errors through rescanning or reindexing when necessary.

  • File medical records in terminal digit order and open additional volumes as needed.

  • Pull and prepare medical records for next-day appointments.

  • Pull and file X-ray films as required.

  • Print daily pull slips, progress notes, and other required forms.

  • Answer phone calls and perform general administrative tasks.

  • Maintain tracking spreadsheets for batches, volumes, and exceptions.

  • Report illegible pages, missing documents, and misfiled records to the supervisor.

  • Handle protected health information in accordance with HIPAA and hospital policies.
