Position Summary:
This position is responsible for collecting, preparing, organizing, and scanning patient medical records into PowerChart, the electronic medical record (EMR) system. This position ensures all documents are accurately indexed and attached to the correct patient record while maintaining productivity standards and protecting patient confidentiality. The ideal candidate is highly organized, detail-oriented, and capable of working efficiently in a fast-paced healthcare environment.
Essential Duties and Responsibilities
Pick up patient charts and medical records from hospital nursing units and departments as needed.
Organize, sort, and group medical record documents for efficient scanning and processing.
Prepare documents for scanning by removing staples, repairing torn pages, and ensuring records are in the correct order.
Scan medical record documents into the PowerChart electronic medical record system.
Verify that all documents are accurately assigned to the correct medical record number (MRN), encounter, placeholder, date, and time.
Perform quality assurance reviews to ensure scanned documents are complete, legible, and properly indexed.
Meet established daily productivity goals, including required page volume targets.
Maintain confidentiality of protected health information (PHI) and comply with HIPAA regulations.
Identify and report document discrepancies or scanning issues for resolution.
Position Qualification:
High School diploma or GED; or one to three months related experience and/or training; or equivalent combination of education and experience.
