Compensation & Benefits:

• $221K–$256K+ FFS compensation with uncapped earning potential

Responsibilities:

• Perform patient assessments and ongoing care management
• Lead quality initiatives and collaborate with facility leadership
• Coordinate care with nursing staff and interdisciplinary teams
• Support operational excellence and patient outcomes
• Maintain accurate, timely documentation

Qualifications:

• Board Certified in Internal Medicine, Family Medicine, or Geriatrics
• South Carolina license and DEA (or ability to obtain)
• Post-acute, inpatient, ED, or clinic experience welcomed
