JOB OVERVIEW
Seeking an experienced physician leader to serve as Medical Director I within a value-based care environment focused on the senior population. This role combines clinical practice, interdisciplinary leadership, care coordination, and oversight of participant care across outpatient, nursing home, hospital, and post-acute settings. The Medical Director I plays a critical role in improving outcomes, supporting care teams, and ensuring high-quality, patient-centered care.

Perform comprehensive participant assessments and ongoing primary care management
Develop individualized care plans in collaboration with interdisciplinary team members
Oversee participant care across outpatient, hospital, nursing home, and post-acute settings
Coordinate admissions, discharges, specialist referrals, and transitions of care
Participate actively in interdisciplinary team meetings and care planning discussions
Provide continuous medical oversight and support for high-risk and medically complex participants
Conduct regular nursing home and home-based visits as needed
Collaborate with community physicians, specialists, and healthcare partners
Support quality improvement initiatives and clinical best practices
Provide clinical guidance and mentorship to Nurse Practitioners and other care team members
Assist in maintaining high standards of clinical quality, safety, and regulatory compliance
Participate in rotating evening and weekend call coverage
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
Successful completion of an ACGME-accredited residency program
Board Certification in Internal Medicine, Family Medicine, or Emergency Medicine
Geriatrics experience or certification preferred
Active and unrestricted Physician License
Active DEA Registration and Controlled Substance Certification, as required
Eligibility for enrollment with Medicare and applicable government healthcare programs
Willingness to participate in rotating night and weekend call schedules
2-5 years of physician experience post-residency
Experience working with geriatric or medically complex patient populations
Experience in primary care, managed care, value-based care, post-acute care, or geriatrics
Experience working within interdisciplinary healthcare teams
Familiarity with chronic disease management and care coordination models
Board-certified physician with a passion for senior care
Strong clinical and leadership capabilities
Demonstrated success in collaborative team-based care environments
Skilled in coordinating care across multiple healthcare settings
Committed to improving quality outcomes and patient experience
Strong communicator who enjoys mentorship and interdisciplinary collaboration
