This role involves coordinating and attending medical, dental, and specialty appointments, maintaining accurate medical information and records, monitoring treatments and medications, and completing required documentation. You will work closely with healthcare providers, team members, and family members as appropriate to support each person's health and well-being.In addition to healthcare coordination, you will help promote independence, community inclusion, and personal growth while ensuring the health and safety of the people we support.What We're Looking ForCandidates must possess a high school diploma or equivalent, Medication Administration Certification, experience supporting people with intellectual or developmental disabilities, and a valid New York State driver's license with an acceptable driving record. Medical Assistant experience and previous work in human services or healthcare settings are preferred.
