Medical Coder

We are seeking experienced HCC Medical Coders to support our client in a fully remote contract position. The ideal candidate will have strong experience in HCC/Risk Adjustment coding, ICD-10-CM guidelines, and clinical documentation review.

Certified medical coders are eligible to apply.

## Key Responsibilities

* Perform HCC/Risk Adjustment medical coding based on clinical documentation.

* Review medical records to identify and accurately code reportable diagnoses.

Apply *ICD-10-CM coding guidelines** and HCC coding methodologies.

* Ensure accurate, compliant, and documentation-supported coding.

* Identify potential coding gaps and validate diagnoses supported by medical records.

* Maintain required standards for coding accuracy, productivity, and quality.

* Stay updated on coding guidelines, regulatory changes, and HCC model updates.

* Collaborate with coding and quality teams when clarification is required.

* Maintain patient confidentiality and comply with HIPAA and applicable healthcare regulations.

## Required Qualifications

* 3-5 years of medical coding experience, preferably in HCC/Risk Adjustment coding.

Strong knowledge of *ICD-10-CM** coding guidelines.

* Experience reviewing clinical documentation and assigning appropriate diagnosis codes.

Good understanding of *HCC/Risk Adjustment coding concepts**.

* Ability to work independently and effectively in a remote environment.

* Strong attention to detail and analytical skills.

* Certified medical coders are eligible to apply.
