Full-Time Medical Case Manager
Location: 3535 Market St.
Summary:
The Outpatient Psychiatric Clinic at the University of Pennsylvania Health System seeks a Case Manager in psychiatry with experience in adult behavioral health disorders. The Case Manager will be responsible for providing culturally sensitive case management to adult persons with behavioral health disorders. Case management will occur primarily within the Outpatient Psychiatric Clinic, however, will require in-depth knowledge of community resources and the ability to network/link/access those resources. The candidate should have experience in case management, including assessment, referral, follow-up, and monitoring, through routine office visits and regular phone contact. The position will support the physicians with the practice to collaborate with outside treating professionals and ensure timely documentation. The position requires that an individual is collaborative, outgoing, and able to multi-task while maintaining a substantial clinical caseload.
