Full-Time Medical Case Manager

Location: 3535 Market St.

Summary:

The Outpatient Psychiatric Clinic at the University of Pennsylvania Health System seeks a Case Manager in psychiatry with experience in adult behavioral health disorders. The Case Manager will be responsible for providing culturally sensitive case management to adult persons with behavioral health disorders. Case management will occur primarily within the Outpatient Psychiatric Clinic, however, will require in-depth knowledge of community resources and the ability to network/link/access those resources. The candidate should have experience in case management, including assessment, referral, follow-up, and monitoring, through routine office visits and regular phone contact. The position will support the physicians with the practice to collaborate with outside treating professionals and ensure timely documentation. The position requires that an individual is collaborative, outgoing, and able to multi-task while maintaining a substantial clinical caseload.

Responsibilities:

• Provides intensive case management, including assessment, referral, and follow-up and monitoring of patients, through routine office visits and regular phone contact.
• • Supports patients in planning for their own health and social service needs by assisting patients in identifying goals.
• • Refers to patients needing more intensive follow-up to appropriate services or agencies.
• • Learns about and makes contact with community resources needed by patients.
• • Facilitates collaboration between the patients' treatment teams.
• • Provides patient navigation services to support patients with medical conditions.
• • Facilitates (or co-facilitates) community health education programs
• • Keeps careful records of all contacts made and follows up as necessary. Keeps accurate records of services received by patients. Routinely uses an electronic health record and other computer systems including e-mail, Internet, and Microsoft Office applications.
• • Attends orientation and ongoing training sessions on patient health and welfare, intimate partner abuse, housing, nutrition, substance abuse, counseling, parenting, and family support issues.
• • Meets with supervisor and treatment team regularly to report on and discuss work.
• • Ensures confidentiality of patient records, sharing pertinent information only with the patient's authorization.
• • Understanding of the Outpatient Psychiatry Practice's mission, goals, and objectives.
• • Able to work independently with a high level of energy and contribute as part of a larger team.
• • Performs duties in accordance with Penn Medicine and entity values, policies, and procedures • Other duties as assigned to support the unit, department, entity, and health system organization

Education or Equivalent Experience:

• Associate\'s degree in social work, Public Health, or related field preferred. (Required)
• Bachelor\'s Degree (Preferred)
• 1+ years\' Experience providing direct services in a community outreach setting. (Preferred)
