The Role
We are seeking a highly organized, proactive, and patient-centered Medical Assistant to support our clinical team with outreach, scheduling, and follow-up.
This is a critical role in ensuring patients feel supported and informed throughout their care journey. You will be the bridge between our patients and providers, helping coordinate care and ensure a seamless patient experience.
Key Responsibilities
Patient Outreach & Engagement
Conduct outbound calls, texts, and emails to engage new and existing patients
Support patient onboarding, including intake coordination and documentation collection
Ensure patients understand next steps and feel supported between visits
Scheduling & Coordination
Schedule and manage virtual appointments across providers and disciplines
Optimize calendars to ensure efficient, patient-centered workflows
Coordinate follow-ups, reschedules, and reminders
Follow-Up & Care Continuity
Track and execute post-visit follow-ups and care plan check-ins
Monitor patient tasks, referrals, and next steps to ensure completion
Escalate clinical concerns to the appropriate team members
Administrative & Clinical Support
Maintain accurate records in the EMR
Assist with documentation workflows and patient communications
Support multidisciplinary coordination across the care team
Qualifications
1–3+ years of experience as a Medical Assistant, Care Coordinator, or similar role
Experience in oncology, primary care, or telehealth strongly preferred
High school diploma or equivalent required; Medical Assistant certification or relevant clinical experience preferred
Excellent communication skills—clear, empathetic, and professional
Strong organizational skills and attention to detail
Comfortable with technology, including EMRs, scheduling tools, and Google Workspace
Understanding of HIPAA and patient confidentiality standards
