Position purpose
Members of this team work remotely as an extension of our GI practices. They reach out to patients who have fallen out of care or are overdue for follow-up and get them scheduled, so practice staff can stay focused on patients in the office. The work is scripted, protocol-driven, and documented in the practice EHR. The role is part of the Specialty Alliance operations team and takes day-to-day direction from the Care-Gap Closure program director.
Responsibilities/Duties/Functions/Tasks:
- Outreach: works assigned patient lists, starting with lapsed patients (seen in the last three years but not the last 12 months, with nothing scheduled) and expanding to overdue labs and procedures as the program grows; places outbound calls and sends practice-approved texts using approved scripts and the written outreach cadence; leaves HIPAA-compliant voicemails.
- Scheduling: books office visits on physician and APP schedule templates under each practice's written scheduling permission (and procedures or labs where the practice authorizes it); captures transportation, escort, and interpreter needs; sends reminders and prep instructions; confirms appointments.
- Attribution: applies the team's source tag to every appointment booked so returns can be credited to outreach.
- Orders and recalls: re-sends or recalls existing lab orders and, where the practice and supervising physician or APP authorize it in writing, enters protocol-based orders; routes anything outside protocol to the physician or APP as a task.
- Documentation: records the outcome of every contact in the practice EHR using the team's fixed disposition list (scheduled, declined, moved, transferred, unreachable, deceased) and updates patient contact information; this record feeds weekly reporting.
- Practice partnership: works with each practice's designated office-manager liaison; escalates scheduling-capacity, access, or insurance barriers to the program director; participates in team huddles, training, and quality review.
- Compliance: follows HIPAA, privacy, and call-consent requirements; makes no clinical decisions and routes any clinical question to the patient's physician, APP, or care team.
- Other duties as assigned.
