As we continue to expand our care management initiatives, we are seeking a dedicated Medical Assistant to support our Remote Patient Monitoring (RPM) Program.
This is an excellent opportunity for a compassionate, organized, and technology-savvy Medical Assistant who is passionate about improving patient outcomes through proactive care coordination and remote monitoring.
Responsibilities
- Support patients enrolled in the Remote Patient Monitoring (RPM) program.
- Educate patients on the use of remote monitoring devices and ensure proper setup.
- Monitor incoming patient health data and escalate abnormal findings to the clinical team.
- Conduct outbound calls to engage patients, provide follow-up, and encourage compliance with monitoring protocols.
- Document patient interactions accurately in the electronic medical record (EMR).
- Coordinate with providers, nurses, and care management staff to ensure timely patient care.
- Assist with scheduling follow-up appointments and addressing patient questions.
- Maintain compliance with HIPAA and all organizational policies and procedures.
Qualifications
- High school diploma or equivalent required; Medical Assistant certification preferred.
- Minimum of 1 year of experience as a Medical Assistant, preferably in primary care, care management, or chronic disease management.
- Experience with Electronic Medical Records (EMR).
- Excellent communication, interpersonal, and customer service skills.
- Strong organizational skills with exceptional attention to detail.
- Comfortable using technology and educating patients on medical devices.
- Bilingual (English/Spanish) is required.
