Overview:

Under the direction of licensed medical and/or licensed nursing personnel, the Medical Assistant Care Coordinator (MA CC) play a vital role in overseeing the comprehensive interdisciplinary care plan for patients throughout their healthcare journey. Medical Assistant Care Coordinators support patients by helping them complete necessary healthcare paperwork, following up on patient needs, managing the durable medical equipment (DME) ordering process, and educating patients about available resources for managing their health. The Medical Assistant Care Coordinator will recognize obstacles to care and guides patients through their healthcare journey with compassion and empathy. The Medical Assistant Care Coordinator collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Responsibilities:

Essential Duties:

  • Assist with the management of healthcare programs that address patients’ needs and improve their health
    outcomes.
  • Guide patients through their treatment plans, educate them about their conditions, and provide information on
    available resources
  • Facilitate communication between staff, patients, and family members to ensure seamless coordination of care.
  • Collaborate with patients to establish health goals and track progress toward achieving them.
  • Coordinate with internal and external social services agencies and programs to ensure that patient receives needed
    services.
  • Assists with referral management process as appropriate to ensure patients receive timely access to care.
  • Assists with durable medical equipment (DME) ordering, management, and follow up process to ensure patients receive their durable medical equipment in a timely manner, understand proper use, and complete necessary follow up visits/care related to the DME item.
  • May assist with scheduling and/or coordinating of outpatient diagnostic/surgical procedures.
  • Collaborates with the patient’s care team including primary care team, specialists, home care, hospital team, and any others involved with the patient’s care to optimize clinical outcomes.
  • Works as a team member, interacting with staff, patients, and providers to communicate and coordinate patient care and workflow effectively.
  • Support patients to increase medical compliance, improve health outcomes, and increase their self-report quality of life.
  • Participates in daily patient care (unit) huddles. Increase patient’s knowledge of medical conditions and improve self-management practices.
  • To enhance the patient experience and to avoid delays in care, as needed will supports provider teams by assisting with completing pre-visit planning (PVP), in-basket management, rooming and wrap up, and clinical duties following established protocols, competency validation, and in accordance with Standing Delegation Orders.
  • Enhances the patient experience by practicing AIDET during each patient interaction.
  • Attending daily patient care (unit) huddles and periodic informational (clinic) huddles, meetings, patient conferences, or planning sessions. These may be related to quality assurance, patient care and other related topics within the clinic.
  • Attends staff meetings and education offerings both in person and via teleconference/online as required.
  • Supports organizational initiatives to promote and maintain a strong positive workplace culture.

  • Demonstrated knowledge of activities associated with the delivery of quality Medical Assistant duties.
  • High level of skill at building relationships and providing excellent customer service.
  • Strong attention to detail and accuracy.
  • Ability to utilize computers for data entry and information retrieval.

  • Excellent organizational and time management skills
Qualifications:

MINIMUM EDUCATION:

  • Graduation from high school or equivalent
  • Medical Assistant Diploma and/or National Medical Assistant Certification
  • Graduation from an accredited Medical Assistant Program or equivalent military healthcare experience

MINIMUM EXPERIENCE:

  • Completetion of a Medical Assistant externship
  • 3 years of Medical Assistant experience, perferably in an ambulatory setting 

REQUIRED CERTIFICATIONS/LICENSURE: Holds and maintains these certifications as a professional. Lapsing/expiration of these
certifications/licensure will result in suspension of work:

  • Basic Life Support (BLS) - Obtained through approved American Heart Association or Red Cross.

PREFERRED EXPERIENCE:

  • Experience with Epic.
