We're looking for an experienced MDS Coordinator to join our Skilled Nursing Neighborhoods and help lead the way our residents, care partners, and interdisciplinary teams work together every day.
This is an on-site role. Our MDS Coordinators are key leaders in our community, bridging clinical accuracy with compassionate, person-centered care.
Position Summary
The MDS Coordinator partners across multiple departments to coordinate the development, completion, and ongoing updates of resident assessments and care plans, in accordance with state requirements and the policies and goals of Christian Living Communities. This role monitors and guides the completion of PPS, OBRA, and CAA assessments, steps into direct nursing care when needed, and supports leadership during surveys and inspections by authorized government agencies.
