Frasier is hiring for a full time MDS / RAI Coordinator. This position will be on-site, full-time
 
Here's what you'll do:
 
  • Lead and coordinate the Resident Assessment Instrument (RAI) process across disciplines
  • Complete and submit Minimum Data Set (MDS) assessments accurately and on time
  • Collaborate with nursing, therapy, and other team members to gather and validate information
  • Ensure compliance with CMS and state regulatory requirements
  • Monitor assessment schedules and follow up to keep everything on track
  • Support care planning by ensuring assessments reflect each resident’s needs
  • Serve as a resource to the team on MDS and RAI requirements
  • Participate in on-call rotation responsibilities 
 
Here's what you'll need:
 
  • Current, active RN license in Colorado (in good standing)
  • At least 3 years of experience as a Registered Nurse, including 1+ year with MDS completion
  • Solid understanding of Medicare, Medicaid, and state/federal regulations related to MDS
  • Working knowledge of ICD-10 coding
  • Experience with electronic health records (PCC experience a plus)
  • Strong computer skills and comfort with clinical documentation systems
  • Clear communication skills and the ability to work effectively with residents, families, and team members

              Compensation

              • Pay Range: $33.30 - $42.45 per hour
              • This position will be on-site, full-time
