MDS Coordinator

QUALIFICATIONS:
Current Massachusetts license as a Registered Nurse
Experience in MDS RAI PPS process preferred
Certification in Nurse Assessment Coordinator or similar preferred

MAJOR TASKS, DUTIES AND RESPONSIBILITIES:
•
Conducts all job responsibilities in accordance with The Salmon Health and Retirement policies and procedures, applicable federal and state laws, and applicable professional standards.
•
Works cooperatively as a member of a neighborhood and professional team and keeps all patient/ resident information confidential.
•
Remains current on all regulations and guidelines related to the RAI/MDS/PPS process. Including but not limited to OBRA, Joint Commission, DPH and all other applicable regulations.
•
Facilitates and ensures that each resident receiving skilled (level 1) receives all required Minimum Data Set (MDS) assessments. Works closely with interdisciplinary team to manage RUGs scores and coordination of ARD dates to maximize reimbursement.
•
Reviews all documentation of new skilled level of care admissions to determine appropriate Resource Utilization Groups (RUG)
•
Works cooperatively with all members of the Interdisciplinary team, in particular skilled therapists, nurses, and physicians to establish appropriate Assessment Reference Date (ARD) to achieve best RUG utilization.
•
Accurately completes or delegates the completion of all MDS assessments of residents receiving skilled level of care services under Medicare or Medicare Advantage programs. Assures the accuracy of MDS assessments done by others.
•
Completes or delegates the completion of the Care Area Assessments (CAAs) and care plan decisions based on the RAI process of residents receiving skilled level of care services.
•
Organizes and schedules care plan meetings and conferences for residents receiving skilled level of care service assuring all members of the Interdisciplinary Team including the resident and/or family have been notified.
•
Submits RAI data and appropriately responds to integrity issues generated by Point Right for all resident receiving skilled level of care services
•
Submits all RAI data to the state data base on a schedule that is compliant with Medicare and OBRA regulations
•
Monitors CASPER/QIES system to download updates and QM data. Works with interdisciplinary team to understand and manage QMs
•
Maintains open and frequent communication with the business office regarding any changes or issues affecting reimbursement. Completes triple check monthly.
•
Ensures that Medicare certifications are completed accurately and in a timely manner.
•
Monitors residents covered under a skilled level of care for significant changes.
•
Acts as a resource for staff, residents and families.
•
Monitors and audits records to assure the completion and accuracy of skilled documentation by all disciplines to support the delivery of skilled level of care services
•
Monitors and assures completion of all required assessments, reassessments and consents including but not limited to Falls Risk, Bladder, Skin, Anti-Psychotic, AIMs, etc
•
Participates in community’s Quality Assurance and Quality Improvement process as requested
