Legacy Health and Rehab is seeking an experienced, organized, and dependable MDS Coordinator to join our skilled nursing team. This position plays an essential role in ensuring accurate resident assessments, individualized care planning, regulatory compliance, and appropriate reimbursement.
The ideal candidate will have strong clinical judgment, excellent attention to detail, and the ability to collaborate effectively with nursing, therapy, social services, dietary, and other members of the interdisciplinary team.
Responsibilities:
- Coordinate and complete accurate, timely MDS assessments in accordance with CMS and state requirements
- Manage the MDS schedule and ensure all assessments are completed within required timeframes
- Coordinate resident care-planning meetings and maintain individualized, person-centered care plans
- Review resident documentation for accuracy, completeness, and clinical support
- Collaborate with the interdisciplinary team to identify resident needs, changes in condition, and appropriate interventions
- Monitor Medicare, Medicaid, and managed-care requirements
- Support accurate PDPM classification and reimbursement
- Track quality measures and assist with quality-improvement initiatives
- Participate in audits, surveys, utilization reviews, and documentation reviews
- Educate staff regarding documentation practices and MDS-related requirements
- Maintain compliance with all federal, state, and facility policies and regulations
Qualifications:
- Current Tennessee nursing license in good standing
- Registered Nurse preferred; qualified LPN candidates will be considered
- Previous MDS experience in a skilled nursing or long-term care setting strongly preferred
- Knowledge of the RAI process, PDPM, Medicare requirements, and long-term care regulations
- Strong organizational, communication, and time-management skills
- Excellent attention to detail and ability to meet firm deadlines
- Experience with electronic health records and MDS software
- Ability to work collaboratively as part of an interdisciplinary team
