    Are you an experienced RN with a passion for coordination, quality care, and leadership? Evergreen is looking for for a dynamic Medicare Coordinator – Registered Nurse to join our Health Center team!

    In this exciting role, you’ll do more than just complete MDS paperwork – you’ll be a key resource, a team leader, and an advocate for excellence in resident care.

    What You’ll Do:

    • Coordinate and manage the Medicare program for the Health Center.
    • Complete accurate and timely Minimum Data Set (MDS) documentation.
    • Serve as a knowledgeable resource to unit staff, promoting best practices and collaboration.
    • Step in as a staff nurse when needed to ensure seamless care delivery.
    • Assist in oversight of day-to-day operations and ensure high-quality service for residents in Creekview Rehab.
    • Participate in the on-call rotation, offering support, consultation, and leadership coverage.
    • Be an active member of the Creekview Leadership Team, contributing to strategic planning and operational excellence.

    What We’re Looking For:

    • A licensed Registered Nurse (RN) with a strong clinical background.
    • Experience with MDS, care planning, and Medicare coordination is required.
    • A collaborative spirit and leadership mindset.
    • Someone who thrives in a fast-paced, team-oriented environment and is passionate about senior care.

    This position is 32 hours per week. The work hours are Monday, Wednesday-Friday 9am-4pm. Tuesday 11am-6pm. On call coverage is every other weekend from 4pm on Friday through 6am on Monday.
