Our guiding motto is leading with integrity, empowering people, delivering outcomes.Job Description:Responsible to assign ICD-10 CM diagnosis codes to the residents' medical conditionsAccountable for case management of Medicare Part A and insurance case managementExpertly assess nursing home residents' needs, and coordinating personalized, resident-driven care based on those assessmentsOversee the patient assessment process, setting assessment schedules and assuring that assessments are done in an accurate and timely mannerMake Recommendations and transmit assessments to the Dept. of Health. Review the results to make policy and procedural recommendations to the management staffCoordinate patient care plan in accordance with regulatory requirements.
