    Deerfield Nursing & Rehabilitation Center is seeking an experienced, detail-oriented MDS Coordinator to join our leadership team. The ideal candidate will have a thorough understanding of the MDS process, Medicare and Medicaid regulations, reimbursement, and interdisciplinary care planning.

    Responsibilities:
    • Coordinate and complete accurate and timely MDS assessments in accordance with federal and state regulations.
    • Ensure compliance with CMS guidelines and RAI Manual requirements.
    • Manage Medicare and PPS assessments while maximizing appropriate reimbursement.
    • Collaborate with nursing, therapy, dietary, social services, and other departments to develop comprehensive care plans.
    • Monitor assessment schedules and ensure all required documentation is completed accurately.
    • Participate in Quality Assurance and Performance Improvement (QAPI) activities.
    • Assist with survey readiness and regulatory compliance.
    • Educate and support interdisciplinary team members regarding MDS documentation requirements.
    • Review medical records for accuracy and completeness.

    Qualifications:
    • Current Louisiana RN or LPN license in good standing.
    • Previous MDS Coordinator experience preferred.
    • Strong knowledge of CMS regulations, Medicare, Medicaid, and PDPM.
    • Excellent organizational, communication, and critical thinking skills.
    • Ability to work independently while collaborating with the interdisciplinary team.
    • Proficient in electronic medical record systems.
