    Description

    We are seeking an experienced MDS Coordinator to join our skilled nursing team in Chicago, IL. This is an excellent opportunity for a detail-oriented clinical professional who is passionate about resident care, reimbursement accuracy, regulatory compliance, and interdisciplinary collaboration.

    The ideal candidate will have a strong understanding of the MDS/RAI process, Medicare and Medicaid reimbursement, PDPM, and state and federal regulations.

    Responsibilities

    • Coordinate and complete the MDS assessment process in accordance with CMS guidelines and regulatory requirements.
    • Ensure timely and accurate completion of MDS assessments, Care Area Assessments (CAAs), and comprehensive care plans.
    • Collaborate with the interdisciplinary team, including nursing, therapy, dietary, social services, and physicians, to develop and maintain individualized resident care plans.
    • Monitor MDS schedules and ensure compliance with all federal and state assessment timelines.
    • Review clinical documentation to support accurate PDPM reimbursement and regulatory compliance.
    • Identify opportunities to improve quality measures, clinical outcomes, and documentation practices.
    • Educate and support clinical staff regarding MDS documentation requirements and regulatory standards.
    • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
    • Stay current with CMS regulations, reimbursement updates, and industry best practices.

    Qualifications

    • Current Illinois nursing license (RN or LPN) in good standing.
    • Previous MDS Coordinator experience in a skilled nursing or long-term care setting is required.
    • Strong knowledge of the RAI Manual, PDPM, Medicare, Medicaid, and CMS regulations.
    • Experience with electronic medical records (EMR) and MDS software.
    • Excellent organizational, analytical, communication, and time management skills.
    • RAC-CT certification is preferred but not required.
